Job Title: Certified Nurse Midwife

Job Summary:

Fair Haven Community Health Care is seeking a Certified Nurse Midwife to join our team. As a Certified Nurse Midwife, you will provide high-quality, patient-centered care to women throughout their reproductive cycle. This includes providing care to normal newborns and peri and post-menopausal individuals.

Responsibilities:

  • Develop a plan of care for each patient, including medical history, physical examination, diagnosis, and appropriate treatment and/or referral.
  • Use all available resources in diagnosis and treatment, such as laboratory and other tests in an appropriate and cost-efficient manner.
  • Confers with other clinicians, patients' families, and other support staff concerning treatment and care of patients when appropriate.
  • Refers those cases which require specialist services, but maintains primary care responsibility to assure continuity of care.
  • Maintains accurate medical records and performs appropriate charting and follow-up on patients.
  • Participates in Medical & General staff meetings and appropriate clinical teams.
  • Participates in the Health Center's Performance Improvement program, in other task groups as needed, and pursues continuing education opportunities to update medical knowledge and assure quality patient care.

Qualifications:

  • Graduation from an accredited school of midwifery.
  • Certified or eligible for certification by American College of Nurse Midwives; if eligible, certification must be received within 1 year of starting employment.
  • At least one year of experience practicing full-scope midwifery.
  • Licensed by the State of Connecticut as a Licensed Midwife.
  • Ability and willingness to serve on the active medical staff of the hospitals used by FHCHC.
  • Qualified in Basic Life Support [required].
  • Bi-lingual in Spanish and English preferred.
  • Experience in an ambulatory or outpatient setting midwifery practice.
  • Ability to relate with warmth and effectiveness to patients and other staff.
  • Experience with Epic EMR preferred.
