Job Title: Certified Nurse Midwife
Job Summary:
Fair Haven Community Health Care is seeking a Certified Nurse Midwife to join our team. As a Certified Nurse Midwife, you will provide high-quality, patient-centered care to women throughout their reproductive cycle. This includes providing care to normal newborns and peri and post-menopausal individuals.
Responsibilities:
- Develop a plan of care for each patient, including medical history, physical examination, diagnosis, and appropriate treatment and/or referral.
- Use all available resources in diagnosis and treatment, such as laboratory and other tests in an appropriate and cost-efficient manner.
- Confers with other clinicians, patients' families, and other support staff concerning treatment and care of patients when appropriate.
- Refers those cases which require specialist services, but maintains primary care responsibility to assure continuity of care.
- Maintains accurate medical records and performs appropriate charting and follow-up on patients.
- Participates in Medical & General staff meetings and appropriate clinical teams.
- Participates in the Health Center's Performance Improvement program, in other task groups as needed, and pursues continuing education opportunities to update medical knowledge and assure quality patient care.
Qualifications:
- Graduation from an accredited school of midwifery.
- Certified or eligible for certification by American College of Nurse Midwives; if eligible, certification must be received within 1 year of starting employment.
- At least one year of experience practicing full-scope midwifery.
- Licensed by the State of Connecticut as a Licensed Midwife.
- Ability and willingness to serve on the active medical staff of the hospitals used by FHCHC.
- Qualified in Basic Life Support [required].
- Bi-lingual in Spanish and English preferred.
- Experience in an ambulatory or outpatient setting midwifery practice.
- Ability to relate with warmth and effectiveness to patients and other staff.
- Experience with Epic EMR preferred.
