Key Responsibilities- Provide Clinical Support: Collaborate with our clinical team to develop and implement educational resources and care coordination protocols for patients.
- Address Patient Concerns: Respond to patients' questions and concerns regarding their health, test results, treatment plans, and prescribed medications via phone outreach.
- Deliver Lab Results: Communicate lab results and provide information on treatment solutions to patients.
- Utilize Clinical Judgment: Prioritize patient care needs using standardized triage protocols and clinical judgment.
- Refer Patients to Appropriate Care: Refer patients to emergency services, primary care providers, or specialists as necessary.
- Document Patient Interactions: Accurately and comprehensively document triage assessments and decisions.
- Ensure Patient Intake Information: Complete and reconcile patient intake information.
- Facilitate Medical Documentation: Ensure accurate and timely medical documentation.
- Provide Follow-up Care: Provide follow-up care with patients as appropriate.
- Index Clinical Patient Records: Index clinical patient records as requested.
- Review and Interpret Immunization Titers: Review and interpret immunization titers and document interpretation according to protocols.
- Complete Chart Audits: Complete chart audits for process and health outcomes related to program goals as needed.
- Education: Bachelor's Degree in Nursing or similar field from an accredited college or university; or equivalent professional experience.
- Licensure: Licensed Registered Nurse (RN); multistate licensure preferred.
- Experience: Five years of professional clinical, nursing, or related experience; virtual/telehealth experience preferred.
- Understanding of Clinical Environment: Understanding of broader clinical environment and alternative venues for care like telehealth.
- HIPAA Regulations: Understanding of HIPAA regulations and privacy policies.
- Technical Skills: Experience using diverse technologies, such as Trello, Slack, and Google Docs. Proficiency in Microsoft Office or equivalent applications required.
- Adaptability: Able and willing to learn/adjust to changes in protocols and/or workflows.
- Time Management: Ability to manage multiple priorities and respond in a timely manner.
- Communication Skills: Excellent interpersonal and telephonic communication skills.
- Organizational Skills: Strong organizational and time management skills; strong attention to detail.
