Key Responsibilities:
  • Conduct regular facility rounds to monitor resident care and identify areas for improvement.
  • Supervise the assignments of aides to ensure proper workflow and efficient use of resources.
  • Monitor medication assistance times to ensure accurate recording and follow-up as needed.
  • Intervene in resident conflicts and behavior issues, facilitating communication to address medical and mental health situations in a timely manner.
  • Ensure all aspects of facility incidents and accidents are completed and reported to the EHP Coordinator/Administrator.
  • Attend to late admissions and re-admissions, ensuring proper paperwork and medication changes are completed.
  • Communicate shift activity via 24-hour report to the Enriched Housing Program Coordinator/Administrator.
  • Facilitate communication for environmental situations, contacting the EHP Coordinator/Administrator as needed and recording details for review.
Requirements:
  • Ability to work effectively as a team member.
  • Valid state LPN license and good standing with the State Registry.
