Job SummaryWe are seeking an experienced Registered Nurse (RN) to join our Ocala team as a Home Health Care Professional. The successful candidate will be responsible for planning, organizing, and directing home care services with a focus on community health education.
Key Responsibilities- Conduct initial assessments of patients and their families to determine home care needs, including a comprehensive physical assessment and history of current and previous illnesses.
- Regularly re-evaluate patient nursing needs and make necessary revisions to the care plan.
- Develop and implement individualized care plans that address the unique needs of each patient, incorporating therapeutic, preventative, and rehabilitative nursing actions.
- Administer medications and treatments as prescribed by physicians and provide direct patient care as defined by the State Nurse Practice Act.
- Counsel patients and their families on meeting nursing and related needs, providing health care instructions as appropriate.
- Identify discharge planning needs and implement prior to discharge of the patient.
- Act as Case Manager when assigned by Clinical Supervisor and coordinate patient care for assigned caseload.
- Associate's or Bachelor's degree in Nursing with one to two years of recent acute care experience in an institutional setting. Two years of home health care experience preferred.
- Must be licensed in the State as a Registered Nurse.
- Current BLS certification.
- Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
- Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist.
- Prolonged or considerable walking and standing. Able to lift, position, or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, or crouching.
- This position requires that the employee is willing and able to meet the physical requirements as necessary.
- Visual acuity and hearing to perform required skills.
