**Job Summary:** We are seeking a skilled Travel Nurse RN Home Health Specialist to join our team at Supplemental Health Care. As a Travel Nurse RN Home Health Specialist, you will provide high-quality patient care and support to patients in their homes, working closely with our healthcare team to ensure the best possible outcomes.

**Responsibilities:**

  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and education on disease management.
  • Conduct thorough assessments of patients' physical and emotional needs, developing personalized care plans to meet their unique requirements.
  • Collaborate with our healthcare team, including physicians, therapists, and social workers, to ensure comprehensive care and support for patients.
  • Document patient progress and outcomes, maintaining accurate and detailed records of care provided.
  • Participate in ongoing education and training to stay up-to-date on best practices and industry developments.

**Requirements:**

  • Current RN license in the state of Louisiana
  • Minimum of 1 year of recent home health nursing experience
  • OASIS/Medicare Certified Visits Experience
  • Electronic Charting experience
  • Valid U.S. Driver's License
