Position Summary:

We are seeking a Certified Nursing Assistant who will be responsible for conducting home health visits. This role is crucial in delivering personalized care and ensuring the comfort of our patients.

Key Responsibilities:

  • Addressing personal care needs as assigned, ensuring prompt attention to patient requests.
  • Maintaining safety standards and using medical equipment properly.
  • Providing personal care services such as bathing, grooming, and changing bed linens.
  • Assisting patients with dressing and feeding as required.
  • Monitoring and recording vital signs, including temperature, pulse, and blood pressure.
  • Observing and caring for patients' skin to prevent any complications.
  • Reporting significant changes in patient conditions to the supervising Registered Nurse.
  • Supporting patients with mobility and exercise as directed.
  • Offering assistance with toileting needs and light housekeeping tasks.
  • Performing basic therapeutic procedures as instructed.
  • Providing respite care for patients and their families when needed.
  • Ensuring the living environment of the patient is tidy and organized.
  • Adhering to all documentation and care protocols established by the organization.
  • Participating in quality improvement initiatives and team activities.
  • Respecting the personal needs and environment of patients and their families.
  • Meeting state training requirements as necessary.
