Siouxland Community Health Center is seeking a dedicated Registered Nurse Wellness Coach to join our team.
Key Responsibilities- Provide comprehensive education to patients regarding chronic disease management and prevention.
- Assist patients in identifying and establishing effective self-management goals, including coordination for goal achievement.
- Engage with insurance providers for preauthorization and precertification approvals related to medical appointments, procedures, and diagnostic tests.
- Maintain meticulous records of all preauthorization activities within the Electronic Medical Record (EMR) system.
- Instruct diabetic patients on the proper use of diabetic meters, insulin administration, and the interpretation of meter results.
- Promote public awareness and education on preventative health practices among SCHC patients by ensuring necessary screenings are completed.
- Collaborate regularly with healthcare provider teams, actively participating in daily meetings.
The ideal candidate will possess:
- A valid RN license; a BSN is preferred.
- Current BLS and Mandatory Reporter certifications.
- Experience in clinical office settings and case management.
- Certification as a Diabetes Educator or the ability to obtain it within six months of employment.
- A self-motivated attitude with a strong background in case management.
