Job Summary:
Eugene Rehabilitation LLC is seeking a highly skilled MDS Coordinator to join our team. As a key member of our clinical operations team, the MDS Coordinator will play a critical role in ensuring the highest quality of care for our patients.
Key Responsibilities:
- Lead the MDS Process: Oversee the planning, scheduling, and revising of the Minimum Data Set (MDS) to ensure accurate and comprehensive assessments of patient care needs.
- Collaborate with Care Teams: Work closely with nursing staff, physicians, and other healthcare professionals to develop and implement individualized care plans that meet the unique needs of each patient.
- Ensure Regulatory Compliance: Monitor and guide the completion of PPS and OBRA assessments to ensure compliance with relevant regulations and guidelines.
- Manage Utilization Review: Oversee the Utilization Review (UR) process to ensure efficient and effective use of resources.
- Lead Care Plan Conferences: Schedule and facilitate care plan conferences to ensure that all team members are aware of patient care plans and can provide high-quality care.
Requirements:
- Nursing Degree: Must possess a nursing degree from an accredited college or university.
- Current RN License: Must possess a current, unencumbered, active license to practice as a registered nurse in this state.
- MDS Certification: MDS Certification preferred.
- Experience with RAI Process: Knowledgeable and experienced in the RAI process, CAAs, RUG categories, and MDS 3.0.
- Electronic Medical Records: Experienced with Electronic Medical Records and computer documentation systems.
