Job Summary

The Licensed Clinical Social Worker - Geriatric Mental Health Specialist is responsible for providing initial evaluations and individual and group therapy to patients. This role requires a deep understanding of social determinants of health and the ability to provide care coordination and management to patients with complex challenges.

Key Responsibilities
  • Conduct thorough interviews with patients to assess their mental status and develop personalized treatment plans.
  • Select and implement evidence-based therapeutic interventions to address patients' developmental and behavioral health needs.
  • Provide individual, family, and group therapies to patients, collaborating with physicians and other healthcare team members to ensure comprehensive care.
  • Ensure accurate and timely documentation of patient progress in the electronic health record.
  • Stay up-to-date with community-based and government services, resources, and regulations to assist patients in accessing necessary support.
  • Provide in-home patient visits as needed, utilizing strong communication and interpersonal skills to build trust with patients and their families.
  • Fulfill continuing education requirements to maintain licensure and stay current with best practices in geriatric mental health.
Requirements
  • Master's degree in social work.
  • Current licensure as a Licensed Clinical Social Worker with at least three years of experience in behavioral healthcare.
  • Experience working in primary healthcare settings is preferred.
  • Bilingual in Spanish is a plus.
