Occupational Therapist Home Health Job Summary

CarePartners Living LLC is seeking a dedicated Occupational Therapist Home Health to join our team. As an Occupational Therapist Home Health, you will play a vital role in evaluating patients, developing care plans, and providing therapeutic care to help patients achieve their goals.

Key Responsibilities:
  • Evaluate patients to identify their needs and develop personalized care plans
  • Provide occupational therapy services to patients in their homes, focusing on improving their functional abilities and independence
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination
  • Participate in the management of resources for patient care and organizational improvement
Requirements:
  • Bachelor of Science degree in Occupational Therapy
  • Current North Carolina license to practice Occupational Therapy
  • Driver's license required
  • BLS/CPR Certification
  • One to two years related experience
Preferred Qualifications:
  • Clinical affiliations with a wide variety of acute care procedures, especially orthopedics, neurology, cardiopulmonary, and care of open wounds
