- Full scope RN licensure
- Experience in managing patients with substance use disorders
- Preferred background in primary care settings
- Strong collaborative skills for effective teamwork
- Ability to perform under pressure and meet deadlines
- Excellent communication and interpersonal abilities
- Proficient in problem-solving and conflict resolution
- Availability during standard primary care hours, Monday through Friday
Key Responsibilities:
- Perform comprehensive nursing evaluations that include social, medical, mental health, substance use, and medication histories
- Deliver ongoing care management, chronic disease support, and patient education
- Work in conjunction with physicians to manage medications for substance use disorders and coordinate patient programs
- Ensure precise and timely documentation within electronic health records
- Engage in proactive population management through outreach and case reviews with addiction specialists
- Provide health education, counseling, and risk reduction strategies to patients involved in interventions
- Maintain a professional environment within a multi-provider healthcare setting
- Advocate for patients across all age groups, utilizing nursing judgment and adhering to practice standards
- Execute tasks within the RN scope of practice and legal frameworks
- Involve patients in the development of their treatment plans
- Implement trauma-informed care and de-escalation strategies
- Comply with patient confidentiality laws, including HIPAA regulations
- Address inquiries and direct questions to the appropriate personnel
Compensation: $112,500 annually
