Key Responsibilities for the Physical Therapist:
· Conduct comprehensive physical therapy assessments in accordance with APTA standards and state regulations.
· Formulate individualized Treatment Plans based on initial assessments and ongoing evaluations.
- Incorporate short- and long-term objectives, collaborative input from other healthcare professionals, and necessary involvement from patients and their families.
· Execute Treatment Plans consistently, modifying them as needed based on patient progress.
· Maintain accurate documentation of patient progress following established medical and home health care protocols.
· Assess mobility, strength, and functional abilities of patients, setting realistic and attainable goals for improvement.
· Communicate patient progress to patients, their families, and the healthcare team involved in their care.
· Develop post-treatment plans that include suitable in-home care services.
· Stay updated with professional knowledge through workshops, literature, and continuous education.
· Collaborate with other staff members to ensure a clean, organized, and effective rehabilitation environment.
Qualifications for the Physical Therapist:
· Proven experience in aligning patient needs with effective treatment strategies.
· Extensive knowledge of various physical therapy techniques, including massage, physical agents, traction, hydrotherapy, and the use of therapeutic equipment.
· Experience in supervising and guiding the work of aides and assistants.
· Familiarity with best practices for equipment use and maintenance.
· Background in recommending assistive devices and follow-up care programs.
· Experience working with diverse patient demographics and conditions.
· Understanding of Occupational Therapy principles and their application in patient care.
· A compassionate approach with the ability to inspire patients to achieve their rehabilitation goals safely.
· Minimum of 2 years of experience in providing physical therapy in a home health care context, along with the following credentials:
o Bachelor’s or Master’s Degree in Physical Therapy from an accredited program.
o Valid Physical Therapy license for the respective state.
o Current Basic Life Support (BLS) certification (including AED).
Compensation Structure:
$175 per SoC Visit
$100 per RoC/Eval Visit
$90 per Regular Visit
$35 per hour for Administrative Tasks
