Position Overview

Key Responsibilities:
  • Operates under the professional exemption as defined by FLSA, with a flexible work schedule.

  • Facilitates resolution of medication-related concerns among pharmacy, nursing, and prescribers, demonstrating proactive judgment.
  • Monitors medication therapies for significant interactions and intervenes when necessary, applying pharmacodynamics and pharmacokinetics expertise.
  • Evaluates and documents the appropriateness of selected medications to achieve desired therapeutic outcomes.
  • Communicates potential medication issues and solutions to prescribers effectively.
  • Serves as a consultant in pharmacokinetics and adverse reactions for healthcare professionals.
  • Participates in therapeutic drug monitoring programs, adjusting therapies based on patient conditions and lab results.
  • Processes medication orders accurately in the Electronic Health Record to maintain precise dispensing and charging records.
  • Maintains comprehensive patient profiles, including relevant health information affecting medication therapy decisions.
  • Oversees the packaging, labeling, and distribution of medications, adhering to established guidelines.
  • Monitors scheduled medication usage and maintains controlled inventories in dispensing areas.
  • Compounds and dispenses pharmaceuticals, including sterile and specialized products.
  • Verifies medication orders for accuracy before delivery to patient care areas, resolving discrepancies as needed.
  • Reviews laboratory data for medication appropriateness and recommends adjustments when necessary.
  • Develops and presents educational programs for medical, nursing, and pharmacy staff, and assists in training pharmacy students and residents.
  • Educates patients and their families regarding specific medications.
  • Responds to emergencies to assist with medication management.
  • Provides timely and accurate medication information to healthcare professionals and patients.
  • Maintains the hospital formulary and promotes formulary compliance.
  • Documents all clinical activities and interventions performed.
  • Collects and maintains required logs and records as mandated by the department.
  • Effectively communicates professional knowledge to patients and healthcare professionals.
  • Engages in quality improvement and medication use review initiatives.
  • Organizes and prioritizes workload efficiently within time constraints.
