About the Role

We are seeking an experienced and compassionate LPN/LVN to join our team as a Private Duty Nurse. As a Private Duty Nurse, you will work one-on-one with patients, providing skilled nursing care and support in the patient's home environment.

Responsibilities

  • Provide skilled nursing care to patients in the home setting, including medication administration, wound care, and vital sign monitoring
  • Develop and implement individualized care plans in collaboration with patients, families, and interdisciplinary teams
  • Conduct thorough assessments and evaluations of patients' needs, identifying areas for improvement and implementing interventions as needed
  • Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive patient care
  • Maintain accurate and timely documentation of patient progress and care provided

Requirements

  • Licensed Practical/Vocational Nurse (LPN/LVN) licensure in good standing in the state where services will be rendered
  • Current CPR certification
  • TB skin test (current within last 12 months)
  • Six months prior hands-on nursing experience preferred but not required
  • Reliable transportation
