Key Responsibilities:
  • Conduct thorough assessments of patients to determine their care needs and develop individualized care plans.
  • Provide skilled nursing care, including administering medications and treatments as prescribed by physicians.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination.
  • Monitor patient progress, make adjustments to care plans as needed, and provide education to patients and families on disease management and self-care.
  • Participate in interdisciplinary team meetings to discuss patient care and develop strategies for improving patient outcomes.
Requirements:
  • NYS Licensed Practical Nurse (LPN) licensure.
  • One year of experience in acute care or home health nursing.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Valid driver's license.
