Job Summary

Key Responsibilities
  • Assessment
    • Initiate assessment of vital signs, orientation to room, allergies, and notification of physician within the timeframe stipulated in unit policy.
    • Integrate all information available and complete initial patient systems assessment within the time frame established by unit policy.
    • Recognize and respond to diagnostic data and initiate appropriate action or intervention as evidenced by documentation reviewed by management.
    • Anticipate and promote patient progress through early and accurate assessment.
    • Collect pertinent data using appropriate assessment techniques.
    • Demonstrate ability to provide safe and competent care for patients undergoing invasive hemodynamic monitoring.
    • Able to provide safe nursing care as per standards of care for patients requiring mechanical ventilation.
    • Determine the priority of data collection based on the patient's immediate condition or needs.
    • Involves the patient, significant others, and healthcare providers in data collection when appropriate.
    • Ensure that the data collection process is systematic and ongoing to identify progress and trends that require intervention.
    • Anticipate and promote patient progress through early and accurate assessment.
  • Outcome Identification
    • Identify expected outcomes individualized to the patient.
    • Derive outcomes from the diagnoses/patient problems.
    • Outcomes are documented in terms of measurable goals.
    • Formulate outcomes mutually with the patient and healthcare providers, when possible.
    • Ensure outcomes are realistic in relation to resources available to the patient.
  • Planning
    • Initiate a written nursing plan of care upon admission.
    • Develop a plan of care that prescribes interventions to attain expected outcomes.
    • Review and evaluate the plan of care, update, and document patient's progress toward resolution of problems on every shift as evidenced by chart review.
    • Develop discharge plan with the physician, patient, and/or family and other available resources during the hospital stay.
    • Ensure that the plan of care reflects current nursing practice.
    • Document legibly pertinent information in the patient's record.
  • Implementation
    • Administer medications within stipulated time frame, observe effects, recognize and report adverse reactions.
    • Initiate, administer, and observe intravenous therapy according to hospital policy and procedure throughout the shift.
    • Demonstrate the ability to prepare, titrate, and monitor vasoactive drugs.
    • Assist physicians with the insertion of invasive monitoring equipment, obtain readings, document, and determine significance of readings.
    • Provide individualized instructions to patient and/or significant others regarding the plan of care on an ongoing basis with written documentation of patient's and/or significant other's ability to verbalize understanding of information taught.
    • Coordinate and implement prescribed procedures to ensure accuracy, timeliness, and patient comfort.
    • Implement nursing orders autonomously on a daily basis.
    • Provide psychological support and motivation to enable patients to function at their optimal level of independence as observed and documented.
    • Set priorities for direct patient care as well as assisting other nurses in establishing priorities.
  • Evaluation
    • Determine level of accomplishment of expected outcomes on a daily basis as evidenced by patient progress record and plan of care.
    • Readjust plan of care based on evaluation of the patient's/families' needs.
    • Recognize the need for aggressive discharge planning based on patient's needs as evidenced by interactions with physicians, Social Services, and other resources.
    • Evaluate the patient's care and progress through reviewing of patient progress record and plan of care.
    • Ensure evaluation is systematic and ongoing.
    • Evaluate the effectiveness of interventions in relation to outcomes.
    • Utilize ongoing assessment data to revise nursing diagnoses/patient problems, outcomes, and the plan of care as needed.
    • Involves the patient, significant others, and healthcare providers in the evaluation process, when appropriate.
  • Professional Accountability
    • Consistently maintain complete confidentiality of all medical, financial, employees, computer, and other sensitive materials, which may violate HIPAA regulations and/or jeopardize the privacy of others.
    • Demonstrate ability to perform in a calm and professional manner at all times.
    • Initiate and direct appropriate interventions as it relates to patient care.
    • Demonstrate capability to correlate theoretical knowledge with clinical practice on a daily basis.
    • Recognize self-limitations and utilize appropriate resources for referral of patients.
    • Demonstrate ability to organize and prioritize assigned responsibilities of patient care on a daily basis.
    • Demonstrate the ability to make shift assignments based on patient needs and allocated staff when assuming resource duties.
    • Act as a role model and maintain professional demeanor at all times.
    • Maintain clinical competencies required to perform duties.
    • Maintain all certifications set forth by institution.
    • Attend meetings as required.
    • Attend all mandatory in-services as directed.
    • Actively participate in developing and maintaining performance improvement activities on the unit and hospital-wide.
    • Acquire and maintain current knowledge in nursing practice.
    • Share knowledge and skills with colleagues and others.
    • Establish good rapport and cooperative working relationships with personnel in the organization.
