Key Responsibilities:
  • Apply Severity of Illness, Intensity of Service, and Discharge Screens/Indicators to evaluate medical necessity
  • Perform admission and concurrent reviews for all payers, maintaining legible documentation as directed by the Utilization Management Plan
  • Educate patients and families on post-discharge needs and alternatives available to meet those needs
  • Refer patients requiring extended care to long-term care facilities and arrange for transfers
  • Collaborate with community agencies to provide services for housing, financial, transportation, psychosocial, educational, home health, and home medical equipment needs
  • Initiate patient/family conferences with the healthcare team to achieve positive outcomes
  • Identify, track, and record Alternative Care Days (ACDs) and intervene to prevent or decrease ACDs as appropriate
