In this vital role, you will be responsible for evaluating and coordinating care for our residents. Your proficiency in the Minimum Data Set (MDS) process will guarantee precise and timely documentation, ultimately leading to improved resident outcomes.

Your Responsibilities:

  • Perform thorough MDS evaluations to accurately represent residents' needs and abilities.
  • Examine MDS data to uncover care plan opportunities and enhance resident outcomes.
  • Work alongside interdisciplinary teams to create and execute personalized care plans.
  • Ensure adherence to regulatory standards while maintaining precise documentation.
  • Apply your strong clinical judgment and problem-solving abilities to meet resident needs.
  • Guide and educate team members on MDS processes and compliance regulations.

Your Qualifications:

  • Current Registered Nurse (RN) license in the state of Michigan.
  • Prior experience as an MDS Coordinator or in a related position is preferred.
  • In-depth understanding of the MDS assessment process and RAI guidelines.
  • Exceptional organizational, communication, and interpersonal skills.
  • Ability to collaborate effectively with a diverse team.
  • Dedication to delivering high-quality care to seniors.
