About the Role

As a Weekend Registered Nurse in Home Health, you will provide personalized care to patients in their homes as prescribed by physicians and adhering to applicable laws, regulations, and agency policies. Your primary focus will be on achieving and maintaining continuity of patient care by coordinating with other healthcare professionals involved in the patient's treatment plan. This role involves working within a Point of Care setting.

In this position, you will make initial and comprehensive nursing evaluations, determine the primary focus of care, and develop an appropriate plan of care within state-specific guidelines. You will also submit timely and accurate documentation in collaboration with physician orders.
