Heart Failure Specialist

Trinity Health Of New England, a leading nonprofit health system, is seeking a skilled Nurse Practitioner or Physician Assistant to join its esteemed Heart Failure team at Saint Francis Hospital and Medical Center in Hartford, Connecticut.
This is an exceptional opportunity to work alongside a talented interdisciplinary team, comprising board-certified Heart Failure Attendings and experienced APPs. As a key member of the team, you will be responsible for:
Daily Responsibilities- Collaborating with the advanced heart failure team to manage patients admitted with heart failure
- Working closely with the interdisciplinary team, including hospitalists, social workers, nurses, cardiologists, and other healthcare professionals
- Providing expert care as an extension of the advanced heart failure physician, including management of diuretics, guideline-directed medical therapy initiation, and cardiogenic shock
- Conducting advanced heart failure consultations and attending daily interdisciplinary rounds
To succeed in this role, you will need:
Requirements- Interest in heart failure care and a passion for delivering exceptional patient-centered care
- Cardiology/Heart Failure experience (preferred but not required)
- Advanced Practice Nurse or Physician Assistant CT state license
