Responsibilities:
- Maintain effective communication with healthcare providers regarding recent changes or requests for orders.
- Ensure the community adheres to all federal, state, and local laws and regulations related to clinical care.
- Collect and report occurrences, and assist in developing/implementing improvement plans as necessary.
- Partner with the administrator and other team members to maintain resident and team member health and safety.
- Collaborate with the pharmacy consultant to provide optimal pharmaceutical services to residents.
- Oversee infection control practices in the community.
- Ensure the safe and efficient distribution/administration of medications, injections, nebulizers, and moderate wound care, as per state regulations.
- Identify ongoing needs and services of residents through the evaluation/service plan process, including documentation for residents with a change in healthcare status.
- Manage the completion of assigned evaluations.
- Partner with other department coordinators to identify, review, and discuss potential changes in service/medication levels and update the residents' service plan as indicated by state regulations.
- Maintain communication with residents' families and/or responsible parties regarding changes in care or health concerns.
- Perform other related duties as assigned by management.
Requirements:
- Associate degree (AA) or equivalent from a two-year college or technical school, or six months to one-year related experience and/or training, or equivalent combination of education and experience.
- Maintain current state license as a Licensed Professional Nurse (LPN) or a Professional Registered Nurse (RN) per state regulations.
- CPR and First Aid Certified.
- Experience in assisted living, home health, or long-term care industries preferred.
- Demonstrate competence in assessment skills, injections/medication administration, follow-up, and triage.
