Job Summary

The Population Health Clinical Pharmacist plays a vital role in supporting providers and patients at Hendricks Regional Health medical practice offices. This position is responsible for designing, implementing, and maintaining medication-related population health initiatives and direct patient care activities.

Key Responsibilities
  1. Maintain expertise in unique aspects of drug therapy for medical practice patients, including factors related to patients of all age groups.
  2. Collaborate with physicians, other providers, and healthcare professionals to develop intervention strategies to maximize medication utilization.
  3. Obtain and/or review patient medication histories and/or prescriber orders to identify potential therapeutic problems, medication therapy contraindications, drug-drug and drug-food interactions, allergies, gaps in medication care, and patient-specific medication formulary status.
  4. Create medication therapeutic plans to assist the patient and healthcare team in establishing and achieving desired therapeutic outcomes and identifying alternative options when necessary.
  5. Develop and implement therapeutic and compliance-based programs and patient-specific medication monitoring plans.
  6. Document interventions and medication-related issues in the patient electronic medical record.
  7. Implement new medication regimens upon prescriber order or collaborative practice agreements.
  8. Interpret medication-related laboratory data and conduct pharmacokinetic monitoring of medications.
  9. Monitor medication therapy to evaluate appropriateness of medication indication, dosing, dosage form, route of administration, and therapeutic duplication potential and consult with prescribers for recommended medication therapy changes or enact changes based on collaborative practice agreements.
  10. Establish and maintain relationships with patients to encourage medication compliance to meet adherence and treatment goals.
  11. Conduct patient education on medication(s) and medication devices.
  12. Provide direct patient care, including point-of-care testing, vital sign monitoring, and limited physical examination as necessary to determine patient response to medication therapy.
  13. Identify and actively monitor for adverse drug reactions and medication errors and document occurrences in the event management system and report to the FDA or ISMP when warranted.
  14. Communicate with prescribers to promote the use of medication formularies through recommendation or implementation based on collaborative practice agreements.
  15. Assist in procurement of non-formulary and/or specialty medications for patients when necessary.
  16. Identify and implement cost-effective alternative medication therapeutic options through recommendations to prescribers or collaborative practice agreements.
  17. Provide medication-related education and both formal and informal medication information to prescribers and other healthcare professionals.
  18. Participate in continuing education, meetings, training programs, and keep up-to-date on information relevant to pharmacy-related population health management.
  19. Produce pharmacy medication utilization reports and conduct medication and/or disease-state-based audits as necessary.
  20. Serve on organization committees as defined by pharmacy department leadership.
  21. Maintain BLS certification and related Hendricks Regional Health maintenance requirements.
Requirements
  1. Bachelor of Science in Pharmacy or PharmD degree from an accredited College of Pharmacy required.
  2. Current valid State of Indiana Pharmacist license required.
  3. Completion of PGY-2 Ambulatory Care residency and BCPS or BCACP Certification preferred.
  4. Ambulatory Pharmacy practice experience preferred.
  5. Excellent written and verbal communication skills.
  6. Proficiency with Microsoft Office applications (Word, Excel, PowerPoint).
