Job Summary:

We are seeking a skilled LPN to join our team at ServiceNet, providing high-quality care to individuals with developmental disabilities and brain injuries in our residential programs.

Key Responsibilities:

Monitor and coordinate healthcare services for program participants, ensuring optimal health and well-being.

Institute appropriate follow-up and communicate medical issues to physicians, nurse practitioners, and other medical licensed providers, as well as Area Director, Nursing RN Supervisor, and Health Care Director.

Document all nursing interventions and changes affecting the individual in daily logs, progress notes, staff communication logs, and Nurses Notes.

Maintain current medication and treatment orders according to MAP standards, noting any side effects or adverse effects and reporting to appropriate personnel.

Assist in training staff in personal care, positioning, feeding, and use of supportive devices, promoting independence and community integration.

Accompany patients to medical appointments and Day Programs, and perform additional assignments as directed by management.

Requirements:

High School Diploma or GED required; College Degree or Certificate preferred. Valid MA Nursing License and valid driver's license with acceptable driving record. Physical ability to perform the requirements of the position. Good communication and computer skills.
