About the Job

We are seeking a compassionate and dedicated Certified Home Health Aide to join our team at Modivcare. As a Caregiver, you will have the opportunity to make a meaningful difference in the lives of our clients, providing them with the care and support they need to live safely and independently in their own homes.

Responsibilities
  • Assist clients with activities of daily living, including meal preparation, light housekeeping, dressing, mobility, and personal hygiene
  • Provide safety, oversight, and supervision of clients to ensure their ability to continue to live at home
  • Maintain open communication between families and healthcare professionals regarding client's physical and emotional condition
  • Document/report any changes in client's health status
  • Ensure highest client safety and well-being
