Job Title: Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at ConcertoCare. As a key member of our Field Team, you will deliver high-quality, patient-centered care to individuals with complex needs in the comfort of their own homes.

Responsibilities:
  • Provide clinical support to our Field Team, working collaboratively with interdisciplinary team members to improve patient outcomes and satisfaction.
  • Develop and implement clinical treatment plans under the supervision of providers and registered nurses.
  • Deliver home- and community-based care to complex patients, including wound care, phlebotomy, and medication review.
  • Conduct health screenings and provide clinical services in the field, including patient status checks and cross-coverage for team members.
Requirements:
  • Licensed Practical Nurse (LPN) certification in the state of employment.
  • Experience providing care for patients with complex needs, including those living with mental illness and substance use disorders.
  • Current CPR and BLS certifications.
  • Average to advanced computer skills, including Microsoft Word, Excel, and Outlook.
  • Ability to travel as needed to locations with limited access to public transportation.
