We are seeking a highly skilled Care Coordinator to join our team at UnitedHealthcare. As a Care Coordinator, you will play a critical role in ensuring that individuals receive the necessary services and support to live independently in their communities.

Key Responsibilities
  • Assess and develop individualized care plans that address the unique needs and goals of each person
  • Coordinate with healthcare providers, community resources, and family members to ensure seamless care transitions
  • Identify and initiate referrals for social service programs, including financial, psychosocial, and community-based services
  • Ensure that care plans include covered services, EPSDT, and value-added benefits
  • Communicate effectively with stakeholders to ensure quality coordinated care and services
Requirements
  • Bachelor's degree in social work, rehabilitation, nursing, psychology, special education, or related field
  • 1+ years of experience working with individuals with long-term care needs
  • Intermediate level of computer proficiency, including Microsoft Outlook and Teams
  • Ability to travel 75% of the time and have a valid driver's license
Preferred Qualifications
  • Licensed Social Worker or clinical degree
  • Experience working with individuals receiving services on home and community-based waivers
  • Background in managing populations with complex medical or behavioral needs
