Job Summary:We are looking for a dedicated and compassionate LPN to join our team as a Case Manager. As a Case Manager, you will be responsible for coordinating patient care and ensuring that patients receive the best possible treatment.
Responsibilities:- Coordinate patient care and develop individualized care plans

- Current LPN licensure in California
