Job Summary

As an Independent Optometrist leasing space inside our Sam's Club, you will play a key role in patient care with respect to eye and vision care, as well as general health and well-being.

Key Responsibilities
  1. Provide high-quality eye and vision care services to patients
  2. Establish your own days of practice, hours of operation, patient schedule, and fee schedule
  3. Have autonomy to manage your practice and make decisions
  4. Have the option to opt into equipment, instrumentation, administrative, and/or opt-tech support services for a fair market value
Requirements
  1. Doctor of Optometry (OD) from an accredited optometry school
  2. Valid state optometry license
  3. Ophthalmologist (MD or DO) from an accredited school
  4. Valid state ophthalmology license
  5. General Liability Certificate
  6. Professional Liability Insurance Certificate
  7. W-9
