About the Role

We are seeking a highly motivated and experienced Nurse Practitioner to join our Evernorth Home-Based Care team in Philadelphia, PA. As a Field Based Nurse Practitioner, you will be responsible for providing comprehensive care to patients in their homes, working closely with an interdisciplinary team to improve health outcomes and quality of life.

Key Responsibilities
  • Perform home visits with patients, conducting comprehensive evaluations and targeting risk factors for disease exacerbations and hospitalizations.
  • Prescribe medications, order labs or diagnostic testing, and make sub-specialty referrals, collaborating with community PCPs and other healthcare providers.
  • Educate patients and/or their families on chronic medical conditions, preventative care, and medication adherence compliance.
  • Coordinate with patient's other providers, including PCPs, specialists, and health plan case managers, to ensure all are working together to achieve the same health goals.
  • Share after-hour on-call coverage with other providers on the team, with limited weekend on-call responsibilities.
  • Clinical documentation of acute and chronic healthcare issues through patient encounters in the EMR system, done in a timely manner.
Requirements
  • Graduated from an Advanced Nurse Practitioner program in Adult, Geriatric, or Family Medicine.
  • Active, unrestricted NP license in Pennsylvania.
  • Ideal experience in Primary Care, Geriatric, Adult, Home Care, or Family Medicine, with at least 1 year of NP experience.
  • Current BLS certification; ACLS preferred.
  • Ability to be credentialed by Medicare, Medicaid, and other Private Insurance Companies.
  • DEA license and State Controlled Substance License upon hire or within 3 months of hire.
  • Electronic Medical Records experience and strong computer skills (MS Office Suite).
  • Strong background/interest in home-based primary care and preventative medicine.
  • Demonstrated ability to identify, diagnose, treat, and document acute and chronic disease in adults, and ability to recognize conditions, characteristics, and behaviors that place patients at risk.
  • Valid state driver's license.
  • Highly motivated, self-directed professional with strong organizational skills and comfortable working independently.
  • Effective leadership and interpersonal skills, facilitating problem resolution and maintaining a professional demeanor in difficult situations.
  • Proven ability to pursue feedback, continuing education, and professional development.
  • Proof of yearly influenza vaccination, Hepatitis B vaccination, and TB testing required.
