Key Responsibilities:- Conduct comprehensive assessments of patients' medical, behavioral, pharmaceutical, and social needs
- Identify gaps in care and barriers to good health, and develop personalized care plans to address these needs
- Coordinate care across multiple settings, including home, hospital, skilled nursing facility, and acute care facility
- Provide clinical oversight to non-licensed support team members and delegate tasks as appropriate
- Monitor and coordinate dialysis interventions, medical management, and other patient care activities
- 1+ years of nursing experience in case management or care management, preferably coordinating care across multiple settings
- ~2+ years of healthcare-related experience
- Current, valid BLS certification from a licensed AHA or American Red Cross training facility or provider
- Strong community engagement and facilitation skills
- Ability to travel throughout the assigned region and conduct home visits as needed
