Skilled Nursing Care Coordinator

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team at The Laurels of West Carrollton.
About the RoleAs a skilled nursing care coordinator, you will be responsible for planning, coordinating, and providing high-quality nursing care services to our residents. This includes assessing their health needs, developing individualized plans of care, and collaborating with our interdisciplinary team to ensure seamless delivery of services.
Responsibilities- Provide safe and accurate medication-related interventions to residents.
- Assess the health status of residents and notify physicians of changes in condition.
- Select and implement appropriate nursing interventions to stabilize resident conditions and prevent complications.
- Supervise the care provided by Certified Nursing Assistants (CNAs) and other team members who care for residents.
To be successful in this role, you will need:
- 1-3 years of experience in a long-term care setting.
- Current Licensed Practical Nurse (LPN) licensure in the state of Ohio.
- CPR certification.
The Laurels of West Carrollton offers a competitive salary range of $55,000 - $70,000 per year, depending on experience. In addition to a comprehensive benefits package, which includes health insurance, 401(k) with matching funds, and more, we offer a supportive work environment and opportunities for professional growth and development.
