AIDS Healthcare Foundation is seeking a highly skilled and compassionate Intensive RN Medical Care Manager to join our team. As a key member of our healthcare team, you will be responsible for providing intensive, coordinated healthcare interventions to assist patients in managing their HIV/AIDS.
Key Responsibilities- Assess client needs and develop a coordinated plan of care to ensure optimal clinical outcomes.
- Collaborate with healthcare providers to coordinate care for comorbid chronic or disease conditions.
- Communicate with patients, families, and healthcare teams to ensure seamless care coordination.
- Provide education and coaching to promote adherence to treatment regimens and self-management of HIV/AIDS.
- Monitor and evaluate the effectiveness of care plans and make adjustments as needed.
- Bachelor's degree in Nursing (BSN) or License Practical Nurse (LPN) with a minimum of 1 year of Case Management experience.
- Knowledge of Ryan White Services is preferred.
- Strong clinical skills and ability to work in a dynamic environment.
