About the RoleAs a Home Care RN, you will be responsible for providing and directing provisions of nursing and personal care to patients in their homes. You will work closely with other healthcare professionals to achieve and maintain continuity of patient care.
Responsibilities- Provide high-quality clinical services within the scope of practice and within infection control standards.
- Develop and revise individualized plans of care and/or service plans according to federal and/or state program requirements.
- Ensure plan of care frequency and duration meets the needs of the patient and initiates plan of care revisions as needed with physician approval.
- Collaborate with other healthcare professionals to achieve and maintain continuity of patient care.
- Current RN licensure in the state of practice.
- At least one year of experience as an RN, preferably in home care or a related field.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
- Current CPR certification.
