Job Summary:

We are seeking a skilled Physical Therapist to join our Home Care team at Carle Health. As a Physical Therapist - Home Care Specialist, you will evaluate and develop treatment plans for patients of all ages, modifying the plan of care as appropriate.

You will serve as a role model in the delivery of professional services and as a clinical resource for staff and students. Hours may vary depending upon census and program need.

Key Responsibilities:

  • Evaluate and develop treatment plans for patients of all ages
  • Modify the plan of care as appropriate
  • Serve as a role model in the delivery of professional services
  • Collaborate with interdisciplinary teams to provide comprehensive patient care
  • Document patient information and treatment plans

Requirements:

  • Bachelor's Degree in Physical Therapy
  • Physical Therapist (PT) License in Illinois
  • One (1) year of experience in healthcare or physical therapy
  • Ability to work as a member of an interdisciplinary team
