Registered Nurse

As a Registered Nurse at Yale-New Haven Health, you will play a vital role in delivering high-quality patient-centered care to our geriatric and adult psychiatric patients. We are seeking a skilled and compassionate nurse to join our team and contribute to the delivery of exceptional care.
Key Responsibilities- Assessment and Diagnosis: Conduct thorough assessments of patients' physical and emotional needs, and develop individualized plans of care to address their unique requirements.
- Implementation and Coordination: Implement care plans, coordinate with interdisciplinary teams, and ensure seamless transitions of care to achieve optimal patient outcomes.
- Health Education and Promotion: Provide patients and their families with education and support to promote healthy behaviors, self-management, and overall well-being.
- Collaboration and Communication: Work effectively with patients, families, and healthcare teams to ensure open communication, respect, and empathy in all interactions.
- Quality Improvement and Safety: Participate in quality improvement initiatives, identify areas for improvement, and contribute to the development of evidence-based practices to enhance patient safety and outcomes.
- Education: Graduation from an accredited school of nursing, with a Bachelor of Science in Nursing (BSN) preferred.
- Licensure: Current registration as a Registered Nurse in the State of Connecticut.
- Experience: 2 years of experience in geriatric or adult psychiatric nursing, with a strong foundation in assessment, diagnosis, and treatment planning.
- Certifications: Current Basic Life Support (BLS) certification, with clinical specialty certification highly encouraged.
