Responsibilities:

Your Role:
In addition to collaborating with an OUTSTANDING Team, here are some of your key responsibilities:


Deliver nursing care and administer medications to clients based on individualized care plans, physician directives, and urgent needs, while accurately documenting clinical observations and progress notes in the client's medical records.

Oversee daily operations for nursing and care staff in partnership with the Center Director.

Conduct assessments and monitor clients' vital signs and responses to treatments and medications, promptly reporting any irregularities to physicians.

Coordinate care with various member services (e.g., Dietary, Pharmacy, Dental, Rehabilitation, Physical/Speech/Occupational Therapy, Audiology, Durable Medical Equipment).


Assist clients with daily living activities, including but not limited to personal hygiene, meals, mobility, arrivals and departures, and social engagement.

Ensure that our clients receive care in a nurturing and positive environment.

Qualifications:
In addition to enjoying your work and caring for our clients, the following qualifications are required:

High School Diploma or Equivalent

Current RN License, CPR certification, and valid malpractice insurance

A minimum of one year of full-time equivalent experience in a licensed healthcare facility.

Experience working with elderly individuals and/or adults with disabilities is highly preferred.

This role requires physical activity, as we are ACTIVE Day. Therefore, you should be capable of:

Performing tasks that involve pushing, pulling, squatting, turning, pivoting, transferring, and lifting at least 50 pounds.

Remaining seated, standing, and/or walking for extended periods while providing physical assistance to clients.
