About the Role

We are seeking a skilled Clinical Social Worker II to join our team at AccentCare, Inc. as a Per Diem Hospice Clinical Social Worker II. As a key member of our interdisciplinary team, you will provide high-quality patient care and support to individuals and their families in need of hospice services.

Responsibilities
  • Assess patient needs and develop individualized care plans to meet their physical, emotional, and social needs.
  • Provide counseling and support to patients and their families to address grief, loss, and end-of-life issues.
  • Collaborate with healthcare professionals to ensure comprehensive care and coordination of services.
  • Document patient progress and maintain accurate records.
  • Participate in interdisciplinary team meetings to discuss patient care and develop strategies for improvement.
Requirements
  • Master's degree in Social Work (MSW) or related field.
  • Current licensure as a Clinical Social Worker in the state of operation.
  • 2+ years of experience in hospice or palliative care.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
