About the Job
The Occupational Therapist contracted through Freedom Home Health And Hospice Care Services Inc. is responsible for implementing standards of care for occupational therapy services and adhering to all conditions outlined in the Service Agreement.
Job Description Summary:
The successful candidate will evaluate patient's functional status, including muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability, work capacity, etc. They will also assess home environment for hazards or barriers to independent living, identify equipment needs, and participate in developing a total plan of care.
Key Responsibilities:
1. Evaluate patient's functional status and develop a treatment program to establish goals for improved function.
2. Perform work capacity evaluation for patients planning to return to work and refer to specialized vocational training as per Organization policy.
3. Develop and communicate a plan of care to referring physicians and other Organization professionals.
4. Teach new skills or retrain patients in daily activities lost due to illness or injury, according to Organization policy.
5. Fabricate splints and instruct patients in using adaptive equipment to improve function.
6. Train patients in prosthetic and/or orthotic devices.
7. Maintain clinical records, notes, and reports to physicians on patient condition changes.
8. Promote coordinated care by maintaining contact with other personnel involved in patient care.
9. Attend inservices, case conferences, and meetings to ensure comprehensive plans of care.
10. Identify patient and family/caregiver needs for other home health services and refer as appropriate.
11. Instruct patient's family/caregiver and other Organization healthcare personnel in patient treatment regimes when indicated.
12. Supervise certified occupational therapy assistants according to Organization policy and state regulations.
Position Qualifications:
1. Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association.
2. Registered with the National Registration Examination of the American Occupational Therapy Association.
3. Two years of clinical experience as an occupational therapist.
4. Good verbal and written communication, and organization skills.
5. Possesses current CPR Certification.
6. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
Salary Information:
We estimate the salary for this position to be around $65,000-$85,000 per year, depending on location and experience.
