Avera is seeking a dedicated and compassionate Registered Nurse to join our team as a Case Manager. As a valued member of our multidisciplinary team, you will play a critical role in promoting quality patient care and facilitating efficient utilization of resources.

Key Responsibilities:

  • Collaborate with the Multidisciplinary Care Team to promote quality patient care
  • Facilitate efficient utilization of resources
  • Identify patient care needs throughout the continuum of care
  • Coordinate care through collaborative practice

Requirements:

  • Graduate from a CCNE or NLN accredited nursing program
  • Licensed as a Registered Nurse in the State of South Dakota and/or meeting the requirements of Multi-state licensure
  • Excellent communication skills

Work Schedule:

  • Monday - Friday, 8:00am - 4:30pm
  • 80 hours every 2 weeks
  • Holidays: on-call rotation
  • On-call: every 6th weekend
