LHC Group is seeking a skilled Physical Therapist to join our team in Los Lunas, New Mexico. As a Physical Therapist with our company, you will play a vital role in delivering high-quality, patient-centered care to individuals in their homes.
Key Responsibilities- Evaluate patients' functional status, medical conditions, and physical therapy needs, and develop personalized treatment plans to improve their well-being.
- Provide physical therapy services within the scope of practice, in accordance with the plan of care, and in coordination with other healthcare team members.
- Monitor patients' progress, report any changes in their condition to the nurse supervisor and/or physician, and adjust treatment plans as needed.
- Collaborate with patients, families, and caregivers to educate them on the plan of care, functional limitations, and progress toward goals.
- Visit patients in their homes, providing services as needed, and documenting progress in accordance with LHC Group's policies and procedures.
- Participate in staff conferences and committees as requested by the agency, and contribute to the development of policies and procedures that promote high-quality patient care.
- Current Physical Therapy licensure in the state of New Mexico.
- Current CPR certification.
- A valid driver's license and access to a dependable vehicle.
