Family Medicine Physician

The physician functions as a Primary Care Provider (PCP) within a community-based outpatient clinic, operating under a collaborative Patient Aligned Care Team Model.
Team StructureWorking alongside a dedicated team that includes a PCP, RN, LVN, and administrative support, the PCP is responsible for managing a patient panel of approximately 1, individuals.
Core ResponsibilitiesThe PCP delivers essential primary care services in a team-oriented environment, focusing on:
- Chronic disease management
- Preventive health care
Common conditions managed include:
- Diabetes
- Hypertension
- Chronic obstructive pulmonary disease
- Degenerative arthritis
- Depression
- Congestive heart failure
- Chronic pain
The physician is tasked with evaluating, diagnosing, and treating non-surgical adult patients across all age groups, addressing a wide spectrum of diseases, injuries, and disorders that are not life-threatening.
In addition, the physician is responsible for:
- Assessing and stabilizing patients with urgent conditions, adhering to medical staff policies.
- Collaborating with and overseeing mid-level providers, such as nurse practitioners.
- Working in conjunction with various specialties, both within and outside the healthcare system, to manage or co-manage patient health effectively.
On-site mental health and subspecialty services are accessible, with additional medical and surgical specialty services available through consultations.
QualificationsThe ideal candidate should be board eligible or board certified in Family Medicine or Internal Medicine, with the VA accepting any valid state medical license.
