Nurse Practitioner

VNA Health Group is seeking a skilled and compassionate Registered Nurse to join our Home Health team. As a Registered Nurse, you will provide high-quality patient care in the comfort of their own homes.

The Registered Nurse is responsible for assessing patient needs, developing and implementing individualized care plans, and providing education and support to patients and their families. You will work collaboratively with our interdisciplinary team to ensure seamless care coordination and optimal patient outcomes.

• Assess patient needs and develop individualized care plans
• Provide skilled nursing care to patients in their homes
• Collaborate with the interdisciplinary team to ensure seamless care coordination
• Provide education and support to patients and their families
• Document patient progress and outcomes

• Active Registered Nurse license
• BSN preferred
• 1+ year of supervised clinical experience as a registered nurse, preferably in home health or med-surg
• Excellent verbal and written communication skills
• Solid interpersonal and time management skills
• Strong computer skills
• Strong problem-solving skills
