  • Coordinate the assessment of patient home health benefits, ensuring medical necessity and ongoing insurance approvals.
  • Continuously evaluate patient needs, ensuring that care is tailored, appropriate, and adheres to physician directives.
  • Oversee the creation, implementation, and regular updates of individualized patient care plans.
  • Manage communications with physicians, clinicians, patients, and referral sources, documenting updates and new orders effectively.
  • Perform additional responsibilities as assigned.

Qualifications:

  • Current RN licensure in the state of Texas.
  • Valid CPR certification.
  • Possession of a current Driver's License, vehicle insurance, and access to a reliable vehicle.
  • Previous experience in Home Health, particularly with OASIS, is highly preferred.
