Patient Care Coordinator Specialist

Key Responsibilities:- Daily scheduling of physician orders, new referrals, and schedules with field clinicians
- Reporting new or pertinent patient information to field clinicians
- Scheduling visits and tasks specific to each patient, ensuring accuracy and compliance with physician orders
- Communicating new physician orders and referrals with contract therapy staff
- Responding to time-sensitive messages from customers, leadership, and team members
- Preparing clinician schedules and assigning clinicians to patients based on treatment frequency and duration
- Demonstrating exceptional customer service to patients, clinicians, and family members
- Participating in on-call rotation for home health, including shared holiday coverage
- Performing general clerical duties in support of patient services
- High School Diploma / GED (or higher)
- 1+ years of experience in a medical setting, performing administrative duties
- 1+ years of experience in a fast-paced environment, requiring multi-tasking
- 1+ years of scheduling experience
- Intermediate level of proficiency in Microsoft Office products
- Ability to work between 8:00am – 6:00pm, Monday-Friday
- Ability to work on call 24/7, one week per month, and 1-2 holidays per year
- Experience with medical terminology
- Attention to detail
- Strong communication skills
