At Optum, we're dedicated to delivering care and improving health outcomes for millions of people worldwide. As a Patient Care Coordinator, you'll play a vital role in connecting patients with the care, resources, and support they need to thrive.

Key Responsibilities:
  • Daily scheduling of physician orders, new referrals, and schedules with field clinicians
  • Reporting new or pertinent patient information to field clinicians
  • Scheduling visits and tasks specific to each patient, ensuring accuracy and compliance with physician orders
  • Communicating new physician orders and referrals with contract therapy staff
  • Responding to time-sensitive messages from customers, leadership, and team members
  • Preparing clinician schedules and assigning clinicians to patients based on treatment frequency and duration
  • Demonstrating exceptional customer service to patients, clinicians, and family members
  • Participating in on-call rotation for home health, including shared holiday coverage
  • Performing general clerical duties in support of patient services
Requirements:
  • High School Diploma / GED (or higher)
  • 1+ years of experience in a medical setting, performing administrative duties
  • 1+ years of experience in a fast-paced environment, requiring multi-tasking
  • 1+ years of scheduling experience
  • Intermediate level of proficiency in Microsoft Office products
  • Ability to work between 8:00am – 6:00pm, Monday-Friday
  • Ability to work on call 24/7, one week per month, and 1-2 holidays per year
Preferred Qualifications:
  • Experience with medical terminology
