Hospice Care Professional Job Description

Cynet Health is seeking a skilled Hospice Care Professional to join our team. As a Registered Nurse (RN) with experience in hospice care, you will provide high-quality patient care and support to individuals in need.

Responsibilities:
  • Provide professional, evidence-based client-centered care for individuals in hospice residence and patient home.
  • Assume a lead role with the interdisciplinary team and work collaboratively with LPN, Social Worker, CNA, Spiritual Advisor, and MD.
  • Complete admission assessments and care-planning across physical and psychosocial domains.
  • Provide symptom and pain control therapeutic treatment, medication as prescribed, and critical thinking skills to recognize changes in condition or mentation.
  • Liaise with Physician/Nurse Practitioner and other Health Care Workers to support clients and families.
Requirements:
  • Graduate of an accredited school of nursing, License in good standing, One year of Experience in Hospice, AHA BLS required.
