Job Description

Job Summary: We are seeking a skilled and compassionate Case Manager to join our team at Della Infotech. As a key member of our healthcare team, you will be responsible for coordinating patient care and ensuring seamless transitions from admission to discharge.

Key Responsibilities:

  • Facilitate the coordination of care with a focus on realistic treatment outcomes within the scope of available resources.
  • Oversee the assessment, planning, facilitation, coordination, evaluation, and advocacy for options and services to meet a patient and family's comprehensive health needs.
  • Collaborate with the interdisciplinary team, patient, and family, as well as various departments within and outside the facility to assure timely and appropriate discharge planning.
  • Develop and implement effective care plans that promote quality and cost-effective outcomes.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.

Requirements:

  • RN, OT, PT, or LICSW license in MA.
  • Previous case management experience, preferably in a skilled nursing facility or rehab setting.
  • Ability to work with flexibility in terms of hours.
