Aveanna Healthcare is seeking a skilled Private Duty Nurse - LPN to join our team of dedicated healthcare professionals.
Job Summary:
We are looking for a compassionate and experienced Private Duty Nurse - LPN to provide high-quality patient care in the comfort of their own homes. As a Private Duty Nurse - LPN with Aveanna Healthcare, you will have the opportunity to make a positive impact on the lives of our patients and their families.
Responsibilities:
- Implement and manage patient care plans in accordance with physician orders and company policies.
- Conduct thorough patient assessments and coordinate care with other healthcare professionals as needed.
- Administer medications, treatments, and therapies as prescribed by physicians.
- Document patient care and progress in a timely and accurate manner.
- Provide education and support to patients and their families on health promotion, disease prevention, and management.
- Collaborate with the care team to ensure seamless transitions of care and optimal patient outcomes.
Requirements:
- Active, unencumbered LPN license in the state of practice.
- Current CPR certification from the American Red Cross or American Heart Association.
- TB skin test (current within the last 12 months).
- One year of prior hands-on nursing experience.
- Reliable transportation.
