About the Role:

We are seeking a highly skilled Palliative Care Advanced Practice Nurse to join our team at AdventHealth Littleton. As a Palliative Care Advanced Practice Nurse, you will provide independent advanced practice nursing care to patients, including assessment, diagnosis, development of treatment plans, patient education, and implementation and evaluation of treatment plans.

Responsibilities:

  • Provides comprehensive physical, psychological, social, spiritual, and cultural assessments through observation, interview, review of medical records, and clinical data.
  • Evaluates patient response and intervenes to ensure optimal patient outcomes.
  • Performs patient rounding with an interdisciplinary team and participates in case conferences.

Qualifications:

  • Palliative care experience desired - inpatient or outpatient.
  • Bachelor's degree, Nursing - Required.
  • Master's degree, Nursing - Required.
  • Basic Life Support - American Heart Association - Required.
  • Registered as Advanced Practice Nurse - Required.
  • Registered on the State of Colorado Advanced Practice Registry - Required.
