As a Home Health Speech Language Pathologist, you'll play a vital role in our care teams, working alongside nurses, physical therapists, occupational therapists, home health aides, and medical social workers to help patients rehabilitate, recover, and regain their independence.

About the Role

As a Home Health Speech Language Pathologist, your responsibilities will include:

  • Evaluating, directing, and providing speech/language pathology services to patients in their homes or facilities
  • Participating in the development and periodic review of the Plan of Treatment and Plan of Care
  • Utilizing professional skills and judgment to assess and treat disorders of speech, voice, language, hearing, and swallowing
  • Administering and interpreting diagnostic tests and applications of therapeutic treatments
  • Observing, recording, and reporting changes in patients' conditions and responses to treatment
  • Providing instruction and training to patients in the use of alternative communication systems when appropriate
  • Counseling and instructing patients, families, and healthcare staff
  • Maintaining and submitting documentation as required by the Company and/or facility
  • Preparing and submitting timely written reports of evaluations, visits, summaries, care plans, care coordination activities, and progress reports
  • Participating in care coordination activities and discharge planning
  • Maintaining the highest standards of professional conduct in relation to confidential information
Requirements

To be successful in this role, you'll need:

  • Meet the education and experience requirements for Certification of Clinical Competence in Speech Language Pathology or Audiology granted by ASHA
  • Minimum of six months' experience as a speech therapist/speech language pathologist
  • Home Health experience a plus
  • Current and unrestricted license
  • Current CPR certification
  • Good organizational and communication skills
  • A valid driver's license, auto insurance, and reliable transportation
