Role Summary:
The Cardiac Care Program Coordinator will leverage advanced clinical expertise to collaborate with healthcare professionals, ensuring exceptional care and coordination for patients within the Cardiac Care Program. The role emphasizes enhancing the patient experience and delivering outstanding service.

Key Responsibilities:
1. Oversee daily operations of the Cardiac Care Program, ensuring effective patient coordination.
2. Act as a liaison among the hospital, Cardiac Care Program, and the Heart and Vascular Institute.
3. Provide follow-up support to patients and healthcare providers.
4. Collaborate with leadership to develop and monitor quality outcomes and patient satisfaction metrics.
5. Conduct assessments of patient needs and facilitate appropriate care resources.
6. Organize multidisciplinary care conferences to promote a collaborative approach.
7. Educate patients and families about procedures and care plans.
8. Participate in policy development and maintain patient care standards.
9. Coordinate diagnostic testing and assist families with logistical needs.
10. Track quality data and participate in performance improvement initiatives.

Qualifications:
Education: Graduate of an accredited nursing program; BSN required, MSN preferred.
Skills: Strong focus on patient experience, proficiency in Microsoft Office, and excellent organizational abilities.
Licenses: Registered Nurse (RN) in Michigan.
Certifications: Basic Life Support Health Care Provider (BLS-HCP).
